- Patient AADHAR Card and Ration Card
- Government Hospital referral letter (For Planned cases only)
- Patient Supportive documents (MRI Report and films, USG report, Blood test report, Consultation sheet) before Admission
- Mobile Number (Which is linked to AADHAR and ration card) is mandatory
